Where is it coming off (Inside/outside, left/right pulley)? Have you called Gary at TDR to ask him?
I finally got the used JR charger install on Saturday and align with engine block(1 washer) and PS pulley, but belt keep coming off SC pulley inward when crank. I use the TDR tensioner fixed pulley all day Sunday to adjust the (+/-) with no hope.

No I have not called Gary yet. When I turned clockwise on the TDR pulley, it would come off SC pulley inward and counter clockwise it would come off PS pulley outward rideside.

Check to see if the pulley is tight. If the bolt on the right pulley is at all loose, the belt pressure will put the pulley at an angle and let the belt slip off.
Note: You do need to loosen this pulley bolt to adjust belt tension, but once the belt is tensioned you need to tighten that sucker down.
The tool you're using to show that these are lined up isn't the right kind IMO (looks like a knitting needle?)
Try and get a metal ruler and use it's edge to see if there's a gap (but it doesn't look like there is).
Got an email from Gary, stated a handful of TDR tensioners that the machined surface on the bolt that mounts to the block bracket was not shaved enough and it caused the fixed pulley to pull the belt back toward the bracket. Hopefully he can get me a new one by this weekend because this is my daily driver.
